Patient Triage
This is a great way for you to contact our practice for non-urgent medical or admin requests. It is very simple to fill out so please do give it a go if you need to contact us.
Although the character count is limited on the form, please try to give as much specific and concise/detailed information as possible. Once received our team will get back to you within three working days, if your form is not fully completed we may ask you for more detailed information before we can process your request.
Our online patient triage system is active between the hours of 8am and 5pm, Monday to Friday. Should you need urgent advice that cannot wait please contact the surgery during opening hours, 111 out of hours or 999 in the event of an emergency.
If you do not have access to the internet or find it difficult to use a computer or smartphone you can call reception instead.
