Long Term Health Condition Reviews
Update about Long Term Condition Reviews
We are making a change to how we invite patients for their Long Term Condition Reviews.
From now on, patients will be invited for their review during their birth month each year. This new system will help us organise appointments more efficiently and ensure everyone receives their review annually.
If you have a long term condition, please keep an eye out for your invitation during your birth month.
As such, there will be a transition period and some patients may be invited earlier than usual during the first year to align with their birth month. We understand this may be frustrating if you’ve recently had a review, but we believe this change will be beneficial in the long term.
Thank you for your ongoing help and support with this and hope to see you in your birth month for your review.
If you are concerned about your long term condition or feel it is worsening please do not hesitate to contact the practice and our helpful care navigators will book you in with the appropriate clinician to discuss this further.
Minor Surgery
We provide a minor surgery service for selected conditions.
Appointments for a minor operation should only be made after an initial consultation with one of the doctors.
Physiotherapy
A doctor may refer you to see a physiotherapist. They offer advice and treat problems regarding muscles or joints.
Podiatry
If you have a problem with your foot care, or an illness (e.g. diabetes) that can affect your feet, please complete a podiatry request form, which can be obtained from the receptionist.
Practice Pharmacist
Our pharmacist provides advice and medication reviews.
Sexual Heath and Contraception
